Albert Road Surgery

Shielding Patients

Use the service below if you need a note for your employer or the Department for Work and Pensions for Statutory Sick Pay (SSP)

Get a shielding note

You will be asked for your date of birth.

You will also need to know your NHS number – this is a 10 digit number you can find on any letter the NHS has sent you, for example 485 777 3456.

This data will be checked to see if you’re on the shielded patients list.  If you are, you will be sent a shielding note.

Your note will be sent by email to the address you used to register with your GP surgery. This should take 1 day.

If there is no email address held for you, your letter will be sent in the post to your home address – this should take 3 working days.

Please make sure the surgery has up to date contact details for you.

This note will be valid for 3 weeks from the date you request the note.

You can also use this service on behalf of someone else.
