Year of Care

Our approach to care for those patients with long-term conditions is changing, in the past you would have attended your appointment and the nurse or GP would have provided you with results of your tests and what you can do to help improve your health.

Our surgery is now providing the Year of Care (YoC), which is about improving care for people with long-term conditions (LTCs). It is about putting people with LTCs such as diabetes, hypertension or asthma firmly in the driving seat of their care, and supports them to self-manage. It transforms the annual review into a constructive and meaningful dialogue between the healthcare professional and the person with the long term condition.

You will still be asked to attend the surgery for a 20 minute appointment with our Nursing team, who will take a blood sample, height, weight and blood pressure measurements and ask you about your general health.  Following this appointment you will be sent a letter containing the results of your blood tests and be offered a follow up appointment.  This appointment may be one of the following:

  • Telephone appointment – this will be with either a practice nurse or a GP
  • Face to face appointment – this will be with either a practice nurse or a GP
  • Self Care Coach – We have coaches who work from our surgery to help support patients manage their health and well being better.  These appointments can last up to an hour each time.  More information about the service can be found here.
